As far as the GPL headers go, should everyone who's ever made a change to that file be included? I'm assuming yes, and working on straightening it out.
Only people who made substantive, non-mechanical changes need to be listed, other changes aren't copyrightable anyway. You can of course list everyone anyway, heh.
Yeah, everyone who contributed a non-trivial amount of code, something like 4 or more (non-trivial) lines of code maybe... Merely fixing typos etc. doesn't make you a copyright holder on (part of) the code.
