[LinuxBIOS] [PATCH] Add missing license headers + fix existing ones (AMD LX)

Hi,

can somebody from AMD please complain if I got something wrong in these
patches? I'm basically adding 2 missing license headers, and doing minor
cosmetic fixes (no content changes) on some existing headers.
