Did this ever work and break again or was it just broken the whole time? […]
I don't know that this ever worked.  I know it wasn't working early on, and I'm not positive when CBFS_MCACHE was enabled.  FMAP_CACHE is (or at least used to be) working, but I haven't worked on ToT in a while, so I'm not sure what's working right now.
