cbfstool/cbfs_image.c: Drop compatibility for old images

Old images should use an old cbfstool. It's too much maintenance to
keep cbfstool compatible with older images so drop code that dealt
with images having a legacy cbfs.

This also drops the possibility to add a cbfs master header as this is
now handled by generating those files directly.
