Kyösti pointed out that there are a few more instances that I didn't catch. This should be pretty complete now except for src/vendorcode/amd where the semantic patch still missed a few things.

So, let's make a call: should I leave src/vendorcode as-is (and remove patches made there in this change)? Clean up on a best effort basis (as-is)? Clean up everything?
