doc: add information about contributing patches
This is copied from the "Development Guidelines" page on the wiki, with some unimportant historical bits removed (description of old branch names that are no longer meaningful) and notes on adding flash chips moved to a new page. Sections are also reordered to (hopefully) more closely match the order in which prospective contributors will care about them.
