Thinking about this further, can the xhci_alloc_pipe() code just check
if the requested pipe was recently free'd (by scanning the free list)
and then reuse it (by calling xhci_alloc_pipe itself)?  If so, it
wouldn't require changes to the usb.c code and it wouldn't introduce
any leak scenarios.
