Looks like an implementation detail that is not used in practice (afaics).
Again, have you ever seen such a layout? I think it's confusing to the reader,
and you call it "common" in the text.

Can't say that I have seen many IFWIs, but I wouldn't call it common if
it's unusual to have a BPDT entry above all S-BPDT entries.
